What to expect when you visit us for the first time.

Starting pelvic floor therapy can feel like a big step. Here’s exactly what happens before, during, and after your first appointment – so there are no surprises.

GETTING STARTED

Where you’ll be seen.

If you’re a patient suffering from pelvic floor pain in Morris County – including Morristown, Madison, Summit, and Chatham – our center is conveniently located nearby. For other patients, we’re happy to schedule a telehealth consultation depending on your needs.

In Person

Visit our Madison, NJ office if you’re located in or near Morris County. Our space is designed to be calm, private, and comfortable from the moment you walk in.

Telehealth

Not local? We offer virtual consultations so you can get guidance and answers no matter where you’re located, on your schedule.

YOUR FIRST VISIT

Four simple steps before you walk in.

1.

Call ahead with your info

Have your prescription and insurance card ready when you call. This lets us verify your insurance benefits and give you an estimate of coverage before you arrive.

2.

Complete your paperwork

Print out and fill in your patient forms ahead of time. It saves you time in the waiting room and gets you into your evaluation faster.

3.

Arrive 15 minutes early

This gives you time to settle in, hand off any remaining paperwork, and start your evaluation calm and un-rushed.

4.

Dress comfortably

Wear comfortable clothing that allows easy access to your problem areas, so your therapist can properly evaluate you.

CHECKLIST

What to bring to your first appointment.

A prescription, if needed

For physical therapy, from your referring physician, ARNP, or PA.

Insurance information

We'll make a copy of your card at check-in.

Completed patient forms

Print, fill out, and bring your patient forms with you to save time before your evaluation.

Claim details, if applicable

In case of an automobile accident or worker's compensation claim, bring any case manager or insurance adjuster contact names, phone numbers, and your claim number.

Two reminders: please arrive 15 minutes early to complete any remaining paperwork, and wear comfortable clothing that allows easy access to your problem areas.
